Requesting Student Records and Transcripts
To request student records or transcripts, please review the official NYC Public Schools FAQ and complete the required form.
How to Submit the Form
1. Download and complete the Student Records Request Form.
2. Review the NYC Public Schools FAQ for required documentation.
Submit by Email
1. Attach the completed form (and any required documents) to an email.
2. Send the email to both guidance counselors listed below and CC the assistant principals. (Alexandria Bolds & Arlene Mcneal)
3. Please include the student’s full name and ID number (if available) in your email.

1. Print and complete the form.
